Daily vs. as-needed tadalafil: what to discuss with your clinician

Tadalafil can be prescribed two ways. This page explains what each involves, which questions tend to matter, and where the decision belongs: in a conversation with your prescriber. Daily and as-needed tadalafil at a glance

The tadalafil tablet label describes two regimens for erectile dysfunction: as-needed, taken before anticipated sexual activity, and once daily, taken at about the same time every day without regard to the timing of sexual activity. Both are legitimate prescribing choices, and the label does not present one as the default.

What daily use involves

Daily use means following a regular prescription: one tablet, once a day, at roughly the same time, with no timing decision tied to sex. According to MedlinePlus, people on the daily regimen may attempt sexual activity at any time between doses. It does not mean you are aroused all day or that erections happen on their own. Tadalafil increases blood flow to the penis during sexual stimulation, and MedlinePlus is clear that it does not increase sexual desire.

What as-needed use involves

As-needed use means taking a tablet ahead of anticipated sexual activity. MedlinePlus describes this as usually at least 30 minutes beforehand, and not more often than once every 24 hours. The label notes that tadalafil improved erectile function compared with placebo for up to 36 hours after a dose. How far ahead you take it, and how often, is set by your prescription; this page offers no calculator or timing rule.

The prescribing label reports that both regimens improved erectile function compared with placebo in clinical trials, which supports both as reasonable options. What the label does not do is rank one above the other.

The regimens use different amounts of medicine on different schedules, and each was studied on its own terms. The useful question is not which regimen is better, but which fits your body, your medicines and your life.

Safety and medication review

A medication review is essential before either regimen. Daily use adds a specific consideration: the medicine is present continuously rather than only on the days you use it.

Other label items your prescriber will weigh:

• Blood pressure medicines and alpha blockers. The label describes an additive blood-pressure-lowering effect and advises that people be stable on alpha-blocker therapy before starting.
• Medicines that slow tadalafil clearance. Certain antifungals and HIV protease inhibitors change how much tadalafil stays in the body, and the label sets different limits for each regimen when they are combined.
• Kidney and liver function. The label says the once-daily regimen is not recommended when kidney function is severely reduced, and tadalafil is not recommended in severe liver impairment.
• Alcohol. Substantial alcohol intake with tadalafil can increase dizziness, headache, fast heartbeat and low blood pressure.
• Sudden vision or hearing changes. Stop the medicine and seek prompt medical attention.

Thinking about changing your schedule?

That is a normal part of treatment, and it is worth raising. It is not a change to make on your own.
