Getting tadalafil online: what to check before choosing care

An online tadalafil service should make the medication, the care process and the full cost clear before you pay. Daily and as-needed treatment are different prescribing choices, so compare more than the price per dose.

Tadalafil is the active ingredient in Cialis and in generic tadalafil tablets. It is prescription-only in the United States, and plenty of services now offer it online. How an online tadalafil evaluation works

A legitimate telehealth service does not sell tadalafil. It offers an evaluation by a licensed clinician, who decides whether a prescription is appropriate. Reputable services describe a process that looks broadly like this.

You complete a health questionnaire

You answer questions about your symptoms, medical history, current medications and goals. Honest, complete answers are what make the review meaningful.

A licensed clinician reviews it

A U.S.-licensed provider reads your answers, may ask follow-up questions, and decides whether tadalafil, another option or an in-person visit is the right next step.

If prescribed, a pharmacy dispenses it

The prescription goes to a licensed pharmacy, which ships it to you. If treatment is not appropriate, a good service tells you why.

Two things should be true of any service you consider. There is no guarantee of a prescription; a service that promises approval is not offering real care. And your primary care clinician remains a good place to have this conversation. Online care is a convenient option, not a replacement for a doctor who knows your history.

Daily and as-needed treatment are different

According to MedlinePlus, approved tadalafil tablets for ED can be prescribed in two ways: taken as needed before sexual activity and not more than once every 24 hours, or taken once a day every day without regard to the timing of sexual activity. These are separate prescribing decisions, not two prices for the same thing.

The regimen affects the strength prescribed, the quantity dispensed each month and how the cost adds up. Some online services advertise a daily program, some prescribe as-needed only, and some offer both after review. Ask which regimen the clinician is actually prescribing before you compare one service with another. What to ask about your prescription

Before you accept a plan, you should be able to answer these questions from the service's own paperwork or a conversation with the prescriber.

Prescription questions to settle before you pay

Medication: generic tadalafil, brand-name Cialis, or a compounded product? What else, if anything, is in it?
Dosage form: a tablet, a chewable or something else? The form changes which regulatory category the product sits in.

Generic tablets, Cialis and compounded products

Online listings often blur three categories that are regulated very differently. Cialis is the brand-name, FDA-approved tadalafil tablet. Generic tadalafil tablets are FDA-approved copies that must meet the same standards for the active ingredient, strength and quality. Clinical information on MedlinePlus, DailyMed or a package insert applies to these approved tablets.

Compounded products containing tadalafil are different. A state-licensed compounding pharmacy prepares them for an individual patient when a licensed provider prescribes them. They can take non-tablet forms, such as chewables, and may combine tadalafil with other active ingredients. Compounded medications are not FDA-approved, so the FDA has not evaluated their safety, effectiveness or quality the way it evaluates approved tablets.

Two practical consequences follow. A compounded combination that contains tadalafil is not "generic Cialis," and should not be described that way. And onset, duration and study figures drawn from approved tablet labels cannot simply be transferred to a compounded product, because the formulation is not the same. Neither category is automatically better; they are just not interchangeable, and a reputable service says which one it offers.

What the clinician needs to know

The questionnaire is not a formality. Tadalafil interacts with several common medications and carries more risk with certain conditions, and the only way a clinician can catch those issues online is if you disclose them. MedlinePlus lists the following as things your doctor and pharmacist need to know before you take tadalafil.

What to disclose in your questionnaire

Every prescription and nonprescription medication, vitamin and supplement you take, especially nitrates for chest pain, riociguat, alpha blockers and blood pressure medicines.
Heart problems, including a past heart attack, heart failure, angina, irregular heartbeat or stroke.
High or low blood pressure, and whether you have been told to avoid sexual activity for medical reasons or had chest pain during sex.
Kidney disease or liver disease.
Any condition affecting the shape of the penis, blood cell problems such as sickle cell anemia, multiple myeloma or leukemia, and eye conditions such as retinitis pigmentosa.
How much alcohol you drink and whether you use recreational drugs, including poppers.

Nitrates and poppers: the interaction that matters most

MedlinePlus warns not to take tadalafil with prescription nitrates or with street drugs containing nitrates such as amyl nitrate or butyl nitrite, commonly called poppers. When to get urgent help

An erection lasting longer than 4 hours, a sudden decrease or loss of vision, or a sudden decrease or loss of hearing needs immediate medical attention. Stop taking the medication and contact a doctor or go to an emergency department. In an emergency, call 911.

A thorough questionnaire that asks about all of the above is a good sign. A form that asks only for your age and a card number is not.

How to compare providers and pharmacies

The FDA's BeSafeRx program gives clear criteria for a legitimate online pharmacy. According to the FDA, a safe online pharmacy requires a prescription from a licensed healthcare professional, provides a physical address in the United States, is licensed by the state board of pharmacy in your state and in the state where it operates, and has a state-licensed pharmacist available to answer your questions. You can confirm a license through your state board of pharmacy.

The FDA's warning signs are just as useful. Be cautious of any site that lets you buy tadalafil without a prescription, offers prices that seem too good to be true, sends unsolicited emails offering cheap medicine, says the medicine will ship from a foreign country, or is not licensed by a U.S. state board of pharmacy. The FDA notes that products from such sites may contain the wrong ingredients, too little or too much active ingredient, or none at all.

Apply the same logic to the prescribing side. Does the service confirm its clinicians are licensed in your state? Can you reach a person by phone or email? Are privacy practices published? Third-party markers such as LegitScript certification and stated HIPAA compliance are worth noting, but they supplement your own checks rather than replace them. Monthly cost versus cost per dose

The most common pricing trick in this category is the per-dose figure. A daily plan shows a low cost per dose because it includes a dose for every day. An as-needed plan may cost less overall while showing a higher per-dose number. Comparing the two on cost per dose tells you almost nothing about what you will pay.

Compare a full month instead, and include everything that gets billed: the visit or evaluation fee, the medication, shipping, and what happens at renewal. Confirm whether the plan is a recurring subscription and how to cancel. Fill in the table from the service's actual advertised or prescribed plan, and note the date, because prices change.

Is tadalafil available without a prescription?

No. In the United States, tadalafil is a prescription-only medication. The FDA lists the ability to buy medicine without a prescription as a warning sign of an unsafe online pharmacy, and products from such sites may contain the wrong ingredient or the wrong amount. A legitimate service always routes you through a licensed clinician first.
